Yr 7 & 8 Immunisation Forms – CHECK YOUR CHILD’S BAG

Today all Year 7 and 8 students, who are at school, have been given immunisation forms to bring home and have their caregivers fill out.   

It is really important that these are returned to school by Thursday 23 February 2023.   

Please fill out the form to advise:

1. whether you want your child immunised or
2. you do not want your child immunised. 

Returning these forms just lets the nurses know that you have made a decision for your child- so if you do NOT want them to be immunised at school please still return them.

You will only be contacted by the Public Health nurse if the form has not been returned to the school.
